Registrar

The Office of the Registrar is responsible for coordinating registration procedures, the preparation, maintenance and security of academic records, grade processing, transcripts, verifications and ordering of diplomas. Explore this page for related forms and information to assist students, alumni, faculty, staff, and other constituents.

Our Mission

The Office of the Registrar provides the Robert Wood Johnson Medical School community with courteous and timely registration and enrollment services. We hold ourselves accountable for maintaining accurate and current information to both internal and external constituents in accordance with the Family Education Rights and Privacy Act (FERPA). We strive to ensure continuous improvement and innovation in meeting the needs and expectations of the populations we serve, and we promote equity and fairness by supporting the development and consistent application of effective policies and processes. Accountability, in all areas, is the hallmark of the office.

Robert Wood Johnson Medical School offers a number of exciting rotations and electives for visiting students, coordinated through the Association of American Medical Colleges' Visiting Student Learning Opportunities.

Resources and Forms

Find resources and forms for requesting transcripts, document certifications for degree verification, enrollment letters, and more.

  • Requests for a degree certification letter or copy of diploma, embossed with the official school seal, can be made via fax, email, mail or in person. This verification is often requested for employers, licensure purposes, etc.

    When sending the request please ensure to include the following information:

    • Name as it appears on school records.
    • Student identification number, NetID and/or date of birth. 
    • Complete address, fax or email address to which the letter should be sent to.
    • Your telephone number and email address (in case we need to contact you)

    All requests will be processed within three to five business days.

  • Requests for an enrollment verification letter can be made via fax, email, mail or in person. This sort of enrollment verification is often requested for health insurance programs, loan deferments, employers, Jury Duty.

    When sending the request please ensure to include the following information:

    • Name as it appears on school records.
    • Student identification number, NetID and/or date of birth.
    • Complete address, fax or email address to which the letter should be sent to.
    • Your telephone number and email address (in case we need to contact you)

    All requests will be processed within three to five business days.

  • All current students and recent graduates. Please review your unofficial transcript to confirm that ALL desired grades and degrees are visible before requesting an official (paper or electronic) transcript.

    Paper Transcripts

    Requests for official academic transcripts can be made online, via mail, or in person. If applicable, there will be a payment fee of $7 per Paper Transcript, $10 per Electronic Transcript. All transcript requests must be submitted via transcripts.rutgers.edu.

    If you wish to make your request via mail or in person, please download and complete the Transcript Request Form. Completed forms must be mailed or delivered to the Registrar's Office; requests must include the payment noted above. Transcripts will be processed within five business days after the request and payment are received. Fees are payable by check or money order addressed to "Rutgers University."

    If you have supplemental forms to submit for inclusion with the academic transcript request, please email them to myregistrar@rwjms.rutgers.edu for processing.

    Electronic Transcripts

    Electronic transcripts are also available for request via transcripts.rutgers.edu.

    If you are submitting an electronic transcript request and have forms to submit along with the request, you will be able to attach the documents to the electronic transcript requests. We will review and process them accordingly.

    ERAS Transcripts

    When applying to ERAS for residency and/or fellowship you can request both transcripts and MSPE (Dean’s Letter) via email at myregistrar@rwjms.rutgers.edu. There is no charge associated with the processing of these documents when applying for ERAS (residency and/or fellowship).

  • If you need to request a duplicate diploma, you must complete and submit the Replacement Diploma Application. Please be advised that this form must be reviewed and approved before you can make the payment of the replacement diploma of $50. This form should be used by previous graduates who are seeking a replacement of their diploma or certificate. After completion, the form should be submitted as instructed in the form.

  • Complete Rutgers' Student Chosen/Preferred Name Application to formally recognize a chosen or preferred name that is different than your legal name.

    The Official Name Change Affidavit form should be used by an individual seeking an official name change for their university administrative records. After completion, the form should be submitted as instructed in the form.

Our staff use a hybrid work model, offering services both virtually and in-person during scheduled office hours. The Office of the Registrar is open:

Monday through Friday from 8:30 a.m to 5:00 p.m.
